You've got no argument from me there, Dennis. Getting a good installer if you're going with spray foam is crucial. Many builders or homeowners don't know how to find one, however, and that's where third party inspections come in. Also, even good installers have bad days, but if someone comes in behind them with a measuring probe and a Blower Door, there will be fewer sub-par foam jobs.  
I've seen this only once, and it was with closed cell foam, but I've heard of it happening with open cell foam, too. I don't know the details, but I've heard it could result from a bad batch of chemicals, improper mixing, or too high a temperature. Whatever the cause, it's not a good thing. The photo below shows how the spray foam pulled away from the studs. A little bit of uninsulated area like that adds up to a lot of heat loss/gain when the whole house has that problem, as it did here.

The video below is from a house near Charleston, South Carolina that I visited recently, and you'll see that the homeowner in this case didn't get his money's worth. The company kept telling me that it was a good job and I had nothing to worry about, even after I'd had 2 other experienced sprayers from 2 different companies visually examine the foam and confirm that large areas appeared to be off ratio. The 3rd sprayer from yet another company, was also outraged because the company who did my attic had failed to vacuum up all of the old cellulose insulation, and he also noticed areas where the foam was shrinking or pulling away, and this was not even 5 weeks after the spraying.

a) Make sure that your house has been well heated prior to installation because the foam expands at different rates as it hits surfaces with different temperatures. If it is cold outside the sheathing may be much colder than the studs consequently the foam may expand more from the sides of the wall cavity creating air pockets in the wall cavity. This can be minimized by an experienced installer.

I'm in the process of redoing my basement and read through the posts. I took out one inch styrofoam covered by 1/4 " beadboard and installed 2X4s and plan on putting in fiberglass batts between them(r-13) with 1/2 in drywall over top of that. Do you think I will see a significat increase in heat control? Should I have done more?... it's a little after the fact, but still curious to know if I will see a big difference based on your experience.

Thank you, Allison. We have a split system unit (actually 5) with the heat being propane. The foam guy said that it needed ventilation, so completely sealing the attic wouldn't be a good idea. It sounds like you are saying that the systems need to be vented out of the attic or replaced with some type of closed system. But I shouldn't bother spraying if the plan is to leave the soffet vents open. Is that correct?

Also, I'm wondering... Is this website is being regularly updated? A local installer (in Oct, 2010) in MA quoted over twice the price you list here. "Open cell alone ~$1.35 per sq ft for R-13. Or a 'hybrid method' using 2inch closed cell for R-13 followed by 1.5inch open cell for R-6. Hybrid is $2.85 per sq foot for R-19 rating". Maybe installers are charging higher prices to profit from those trying to meet government energy rebate by Dec 31?

Never spray foam on a surface with a moisture level of more than 20 percent—the foam won’t stick. If the surface you’re spraying hasn’t been exposed to water for a few weeks, it should be OK to spray. But if you’re patching a wall that recently sustained water damage, or was just assembled with new, wet lumber, you may want to test the moisture level before you spray. I am building a house in Central Texas (Caldwell). Several builders are cautious about using foam insulation and/or a closed attic. I would like to use both. Here in Texas, heat and humidity (except for the past few years of drought) are a continuing problem. Which type of foam would be the best to use in our home, where should the vapor barrier be or should be use one at all, if we are using fans in the exterior walls to supply fresh air to the house, do we need a vented attic or will it cause more problems than it solve? I have printed out your article and the blogs to give to my contractors and architect, but I would really appreciate your comments on the products being used in my part of the US. 
Here's a simpler, less expensive alternative: Cut some 2-inch-thick rigid-foam insulation and glue it to the subfloor between the joists or support it with nails driven partway into the joists. Then fill any gaps between the edges of the foam boards and the joists using the canned spray foam sold at home centers or hardware stores. An even easier option is to nail the foam panels against the bottom edge of the joists and seal the joints with canned foam, but you'll lose some headroom and access to any pipes and wires between the joists. As with spray-foam kits, protect yourself and the floor from the dripping globs of canned foam. A full face shield, gloves, and a hat would be a good start.

And, I think that the amount of segments-per-season seems just about right - any more and it might begin to take the shows into an area that moves away from their core meaning (i.e. - home repair/rehab/construction, etc). While coatings may potentially stop minor leaks, the roof should be properly repaired and dried prior to coating application. Coatings may be able to seal pinhole leaks, which are leaks not visible to the naked eye.
